(Dr. Sunitha Mahesh, Medical Director & Senior Consultant, Milann Fertility Center, Bengaluru)

Pregnancy relies on a series of intricate steps, one of which is the movement of a fertilized egg (embryo) to the uterus for implantation. In an ectopic pregnancy, however, the embryo mistakenly implants outside the uterus, often in the fallopian tube, which isn’t designed to support an embryo. This serious condition may occur roughly in 2% of pregnancies and can cause severe complications like internal bleeding, and severe infection, requiring urgent medical attention. Ectopic pregnancies cannot progress as normal intrauterine pregnancies, which makes early detection and prompt treatment vital to prevent life-threatening outcomes.

Early symptoms of an ectopic pregnancy can resemble those of a normal pregnancy which include vaginal bleeding, lower abdominal pain, and back pain, as well as weakness or dizziness. These symptoms may intensify in the event of a fallopian tube rupture along with other symptoms like shoulder ache, fainting, extreme pain, and rectal pressure. At times severe lower abdominal pain could indicate rupture of the fallopian tube, necessitating immediate medical attention. If any of these symptoms are noted, please consult your doctor for evaluation.

The cause of ectopic pregnancy is often unknown, but previous scarring or inflammation of the fallopian tubes, hormonal imbalances, genetic issues, and structural abnormalities in the reproductive organs can be probable causes for ectopic pregnancy. Risk for ectopic pregnancy increases in women with age >35yrs, smoking, history of ectopic pregnancy, history of pelvic surgeries, endometriosis, pelvic inflammatory disease, sexually transmitted infections, or fertility treatments, conception despite an intrauterine device or tubal ligation. If you have these risk factors, consult your doctor to discuss prevention strategies.

Ectopic pregnancies pose serious risks to the mother with no viable pregnancy, necessitating prompt medical treatment to protect the mother’s health and future fertility. Treatment varies based on the pregnancy’s location and stage. Medication like methotrexate can stop the growth of the ectopic pregnancy in the early and unruptured stage and when the mother is in stable condition, potentially avoiding surgery. If surgery is needed, a laparoscopy is performed which includes removing that part of the fallopian tube in which pregnancy developed and repair of the tube is done. Post-surgery care includes watching for signs of infection, avoiding undue strenuous activities, and avoiding pregnancy for 3 months.

You can reduce the risk of ectopic pregnancy by abstaining from tobacco, maintaining a healthy weight, regular health checkups, detecting pelvic inflammatory disease in its early stages and getting treated, engaging in safe sexual behavior, and avoiding STIs. Before attempting to conceive, discuss any risk factors with your healthcare physician.

Although ectopic pregnancies cannot be avoided, early detection and prompt treatment results in better outcomes. The long-term outlook is usually positive, with many women having healthy pregnancies after an ectopic pregnancy, but women with existing reproductive issues may face challenges. Support groups and self-care can help manage the emotional impact of pregnancy loss.

By: Dr Sulbha Arora, MD, DNB, Clinical Director, Nova IVF Fertility, Mumbai

In a country where traditions, family values, and societal norms play a prime role in shaping lives, infertile women often face severe social stigma. Many women believe that they bear a higher responsibility for infertility than men, which puts them under greater psychological stress and more prone to suffering severe social consequences. The stigma of not giving birth to children affects approximately 53% to 64% of female infertility patients worldwide. This stigma not only causes harm to the mental health of these women, but also affects their quality of life, making them bear the adverse social consequences such as domestic violence, marriage breakdown, or even delay in receiving the treatment.

According to a WHO report, the prevalence of primary infertility in India was 3.9% (age-standardized to 25–49 years) and 16.8% (age-standardized to 15–49 years) using the “age but no birth” definition. Therefore, it is crucial to have a deep understanding of the patients’ stigma and effective intervention in alleviating it.

In India, the discussion of infertility as a circumstance in both men and women—has just recently gained recognition. A double-digit increase in infertility has made it a personal and societal health concern in India. Infertility negatively affects couples socially and financially, in addition to emotionally. India has a population of as many as 27.5 million infertile people, including men and women, according to the Indian Society of Assisted Reproduction.

Several lifestyle factors contribute to infertility, with low sperm count, lifestyle choices, alcohol consumption, insufficient sleep, delayed marriage, and the dilemma between pursuing a career or getting married being among the key influencers. These elements collectively play a significant role in affecting reproductive health and fertility. It’s essential to recognize the complex nature of infertility, as addressing these factors can enhance the chances of achieving successful conception.

When a couple has regular, unprotected sexual encounters yet is unable to conceive, the couple is said to be infertile. Around 30% of known cases of infertility contain defective sperm, while 40–50% are primarily caused by abnormalities of the female reproductive system. One of the biggest risk factors that can impact someone’s fertility is their age. A man’s fertility begins to fall after the age of 50, whereas a woman begins to decline after the age of 30 and continues to do so after that.

Low sperm count, influenced by various environmental and genetic factors, is a critical aspect of male infertility. Additionally, individuals’ lifestyles, characterized by stress, poor dietary habits, and lack of exercise, can contribute to reproductive challenges. Excessive alcohol consumption is known to impact fertility adversely, as it can disrupt hormonal balance and impair reproductive function. Inadequate sleep, often overlooked, can also be a contributing factor, as it affects the overall well-being of individuals, including reproductive health. Delayed marriage may pose challenges, as age can impact fertility in both men and women.

Furthermore, the modern dilemma of choosing between career aspirations and marriage can add another layer of complexity to the fertility equation, as individuals may face difficulties in finding the right balance. Addressing these factors through lifestyle modifications, timely medical interventions, and open communication about reproductive health can significantly contribute to improving fertility outcomes.

Infertility stigma can bring heavy mental pressure and psychological burden to female infertility patients and affect their quality of life. Therefore, effective and targeted psychological interventions should be developed to reduce the patients’ stigma and improve their quality of life.

BENGALURU, DECEMBER 04 – Thalassemia, a genetic disorder affecting hemoglobin, presents a significant health challenge in India. With the world’s largest population of Thalassemia Major children, ranging from 1 to 1.5 lakhs, and around 42 million carriers of the beta thalassemia trait, the country is struggling with a substantial burden of this condition. Thalassemia was included under the Person with Disability Act as it is a lifelong disease and one where constant care and monitoring are required.

As the most prevalent genetic disorders globally and in India, hemoglobinopathies, particularly Thalassemia Major, require focused attention. Thalassemia patients are mostly children who go through painful blood transfusions for several years. Continuous blood transfusions and iron chelation therapies have their challenges and risks for the patients. The implication of continuous blood transfusion and other therapies impacts the patient and their families and has a huge social and economic burden on the nation.

Guidance related to treatment, financial support, and planning are the most critical factors affecting access to transplantation in India. To combat this challenge, DKMS BMST Foundation India has come up with a Thalassemia Programme for such patients.

Dr. Sunil Bhat, Director and Clinical Lead Pediatric Hematology, Oncology, and Blood and Marrow Transplantation at Narayana Health Group of Hospitals, said, “In view of International Day of Persons with Disability, it’s crucial to spotlight the challenges faced by Thalassemia patients in India. India is called the Thalassemia Capital of the World, as every year, more than 10,000 children are born with this genetic disease. Addressing the Thalassemia challenge in India requires a comprehensive approach, and stem cell transplantation is a cornerstone in this endeavor. It offers a real chance at a healthier life, disrupting the course of this genetic disorder and bringing newfound hope to individuals and their families. Thalassemia is a preventable genetic disorder, and prevalence has been brought down drastically in many countries with effective prevention strategies.”

Patrick Paul, CEO of DKMS BMST Foundation India, said, “The DKMS-BMST Patient Funding Programme and the DKMS-BMST Thalassemia Programme, integral to the DKMS mission, strive to enhance accessibility to transplantation for underprivileged patients in India. Collaborating with local NGOs and transplantation clinics, the Thalassemia program organizes events facilitating free HLA typing for pediatric patients and their siblings. DKMS Germany covers the associated costs. This initiative extends support to cases without matching siblings, aiding in an unrelated donor search. Since 2018, over 14,800 Thalassemia children and family members have received free HLA typing, leading to 345 successful transplants and offering a transformative impact on their diagnostic accuracy and potential for stem cell transplantation.”

In the battle against Thalassemia, survivor Gurnaz Kaur, diagnosed in 2013, was struggling with monthly transfusions and Iron Chelations; her parents sought a lasting solution. Consulting Dr. Joseph John from CMC Ludhiana, they embraced the hope of a blood stem cell transplant. Gurnaz, part of DKMS-BMST’s Thalassemia program, found a perfect match in her elder brother. The transplant, facilitated by CMCL and DKMS-BMST, unfolded a new chapter for her. Two years post-transplant, Gurnaz Kaur now revels in a healthy life, a testament to the significant impact of the program on her well-being.

The life of Gurnaz Kaur and patients like her are true inspirations. DKMS-BMST works with 18 partners across India, like CMC Ludhiana and Narayana Health, to name a few, to continue the mission of supporting more patients under the Thalassemia program. With a vision to identify a matching donor within the family as well as support unrelated donor searches for patients who do not find a suitable match within their family.

The UN designates December 3rd as the International Day of Persons with Disabilities and aims to raise awareness and understanding of various disability issues and mobilize support for the dignity, rights, and well-being of persons with disabilities. Thalassemia, as a disability, can be cured with a stem cell transplant, but there is only a 1 in a million chance that someone comes up as a match for a patient, and Indian patients mainly require an Indian tissue match. Blood stem cell donation is a completely safe process, and more and more young Indians need to come forward and register as potential donors.

To register as a potential blood stem cell donor, please visit www.dkms-bmst.org/register

88% of Corporate India feels the pinch, but is ready to absorb the cost

46% of respondents realise the need for better health support at workplace

Mumbai, [November 30, 2023]: The Annual Health and Benefits Survey 2023 conducted by Global Insurance Brokers (GLOBAL) presents interesting insights into the impact of surging healthcare expenses on Corporate India in the post-COVID-19 landscape. This comprehensive survey in 2023 reveals that an overwhelming 88% of Corporate India is struggling primarily due to the escalating hospital bills.

Key findings underscore a stark reality – healthcare expenditures have surpassed revenue growth, economic inflation, and wage increases, intensifying financial challenges for organisations. The upswing in hospital bills and related expenses has played a significant role in driving up healthcare costs. Additionally, the increasing expenses associated with treating lifestyle diseases have further compounded the financial burden of medical spending.

Beyond financial concerns, the survey spotlights growing apprehension about employee mental health, with 46% of respondents emphasising the pressing need for better mental health support in the workplace.

Mr Prabodh Thakker, Chairman of Global Insurance Brokers, expressed his perspective on these findings: “COVID-19 has led to significant changes in workforce engagement worldwide. The customary work approaches have evolved into hybrid and remote arrangements, propelling modifications in business frameworks, and reforming the dynamics of employer-employee interactions.”

Companies now have access to a diverse range of health and wellness programs to address various facets of promoting and maintaining health. Importantly, today’s employers possess a holistic understanding of well-being, encompassing physical, mental, social, and financial dimensions. However, Ms Puja Parekh, President of Global Insurance Brokers, believes, “Wellness must be tackled strategically and integrated into the overall organisational purpose framework. An open and dynamic workplace character can boost teamwork and mutual trust, leading to lower stress levels and improved mental health conditions, helping organisations reap meaningful business rewards”.

Mr. Naveen Kumar Midha, Executive Director of Global Insurance Brokers, highlighted, “Amidst tripled economic inflation, organisations face a critical challenge: balancing finances while retaining talent. The aftermath of COVID-19 has left uninsured employees financially vulnerable, amplifying the needs of health and benefits decisions. To nurture a diverse workforce and adapt to their evolving needs, the corporate benefits policies must focus on well-being and competitiveness, keeping a tight control on the budgetary constraints.”

He added, “Post-pandemic approach of corporate India has taken a human-centric approach, as claims efficiency, service quality, surging healthcare expenses, and mental well-being impact takes precedence.”

Other key findings of the Survey:

o 32% of Indian employers claim relative immunity to the pandemic, while 47% express significant concern about claims settlement

o 43% are apprehensive about servicing levels provided by insurance companies and third-party administrators

o An exclusive reliance on insurance negotiations as a cost-containment strategy is seen as potentially more expensive

o 58% of employers expect the Medical claims ratio to decrease in 2023

o 78% find Indian insurers performance as satisfactory

o 85% expect insurance brokers to streamline claims settlement issues

o 70% of employers value insurance intermediation services, while 8% express dissatisfaction

o A top TPA’s (Third-Party Administrator) performance was lauded,, with 90% for claims settlement efficiency and 86% for cashless procedure efficiency

GLOBAL’s comprehensive survey included decision-makers from diverse industries and company sizes. The overwhelming response highlights the critical importance of addressing evolving employee expectations and reviewing benefit programs to ensure effective employee engagement and sustainable positioning.

The report calls for the implementation of smart cost-management strategies and innovative infrastructure to address the rising healthcare costs effectively. It also underscores the need for insurers, brokers, and third-party administrators to adapt to the evolving employee benefits landscape.

Mumbai, November 23, 2023: In their latest report, global real estate consultancy Knight Frank and their U.S. partners Berkadia, note that India is currently facing a deficit of 2 billion square feet of healthcare space to cater to its current population base of 1.42 billion people. The estimated need for additional beds to reach the recommended ratio of 3 beds per 1000 people is stands at 2.4 million beds. India has a considerable gap between the number of hospital beds available in the country and the number of hospital beds required. India’s existing bed to population ratio is 1.3/1000 population (both private and public hospitals included), and there is a deficit of 1.7/1000 population. To cater to the existing population, there is an additional requirement of 2.4 mn beds. This disparity provides an opportunity for public and private players, to expand their footprint in the healthcare industry in India. As per Indian government estimates, there are about 582 investment opportunities in medical infrastructure including hospitals valued at US$ 32bn. The hospital industry accounts for 80% of the healthcare market in India. Currently, India has an estimated 70,000 hospitals of which the private sector constitutes 63% of the total share.

India’s healthcare market was estimated to be at US$ 372 bn in 2022, a significant growth from US$ 73 bn a decade ago, in 2012. At this level and pace, India’s health care industry has grown at an annual average rate of 18% and got accelerated with pandemic as it challenged the healthcare sector by stretching the need for infrastructure and service delivery, therefore making it a trigger for transformation. Global investments in healthcare related real estate has reached US$38 bn, accounting to 4.3% of total global real estate investments

Expansion of India’s healthcare market

Source: IBEF, Knight Frank Research

As part of the Sustainable Development Goals, India is committed to achieving Universal Healthcare Coverage. To achieve its goals, the policy makers in India have introduced schemes over the years pertaining to insurance as well as access to affordable healthcare services. India’s National Healthcare Policy (2017) aims to achieve government spending on healthcare upto 2.5% of the GDP. The Central Government’s budgetary allocation to healthcare so far has increased from 1.2% of the GDP in FY 2014 to 2.1% of the GDP in FY 2023.

Demand drivers of India’s healthcare industry

As per Knight Frank’s analysis, India is one of the fastest growing economies with high personal spending potential of individuals of which healthcare comprises as major share. Factors such as the gradual rise in an ageing population, increasing per capita incomes, growing health awareness and the penetration of health insurance drive the demand for the healthcare industry in India. Additionally, there has been a growing incidence of lifestyle diseases in India led by cardiovascular diseases, which will heighten the demand for specialized healthcare.

India: Popular destination for healthcare tourism

India is emerging as one of the most affordable healthcare destinations in the world providing quality medical procedures at a relatively cheap cost. In the pre-pandemic years between 2014-2019, the inflow of foreign tourist arrivals on medical visa grew at a CAGR of 30%. It is noteworthy that despite the deficit in healthcare infrastructure in India, the country is an attractive market for healthcare tourism. As per Medical Tourism Index (2020-21), India ranked 10th out of 46 destinations in the world. With an aim to improve medical tourism, India’s tourism ministry formulated a national strategy and roadmap for medical and wellness tourism in 2022 under the ‘Heal in India’ initiative with the primary objective to strengthen the healthcare ecosystem in the country.

Shishir Baijal, Chairman and Managing Director, Knight Frank India said “The challenge of India’s population to bed ratio, coupled with increasing population, underscores the pressing need for significant enhancement in the country’s healthcare infrastructure. Addressing this demand necessitates nearly doubling the current real estate capacity to adequately meet the expanding healthcare requirements of the population. Post the pandemic, healthcare ranks amongst the mainstream avenues for investors looking for investments into long income generating assets. According to our Attitudes Survey, almost one-fourth of the Indian ultra-high net worth individuals had cited to invest in healthcare related assets in 2023. There is also an emerging opportunity for investments in medical research and development, to accentuate discovery of new drugs to navigate any future break out of pandemic inflicted diseases.”

The global healthcare sector remains buoyant and is expected to grow despite a challenging economic outlook. This comes as the global population shifts rapidly towards an ageing demographic, which is anticipated to drive demand for elderly care beds, particularly for full-time nursing care delivered in specialised facilities. Investors have seized upon is trend, with North American capital contributing almost 68% of the funds deployed in the past year, while France and Belgium attracted the highest levels of cross-border investments. Recognizing the potential of APAC’s ageing demographic, the private sector is actively exploring opportunities in response. Acquisitions of operating assets in the APAC care sectors reached a record high of US$2.8 billion in 2022.

Julian Evans, Partner and Head of Healthcare at Knight Frank said: “Broad in its coverage, the case for investment in the healthcare sector remains consistent not only across its various subsectors but also across geographic borders. Supported by an ageing population across the globe and shifting demographic trends, the healthcare sector is seeing increased demand for long-term care facilities. Furthermore, as private equity, REITS and institutional investors continue to chase the strong, long income generated, there is growing interest in healthcare’s capabilities to aid ESG investing strategies.”

With several cases of tomato flu being reported in children in Kerala, Karnataka Health Minister K Sudhakar has instructed authorities to keep a vigil on daily travellers from the state entering border districts like Mangaluru, Udupi, Kodagu, Chamrajnagara and Mysuru. Sudhakar also reiterated that there was no need to panic.

Karnataka Health Minister K. Sudhakar on Wednesday said that there is no need to panic against the backdrop of an outbreak of Tomato Flu in neighbouring Kerala.

“Though some symptoms are similar to Covid-19, the Tomato Flu has nothing to do with Covid-19. These symptoms are usually seen in other types of viral infections also.

“There is no need to panic as the authorities have been asked to remain alert. Moreover, the Tomato Flu is endemic to Kerala,” he said.

According to a release by the Health Department, Tomato Flu is a rare viral disease, which causes red-coloured rashes, skin irritation and dehydration. The disease gets its name from the blisters it causes, which look like tomatoes.

Tomato fever is affecting children below the age of five in Kerala, it said, adding that the main symptoms include large blisters the size of tomatoes which are red in colour, as well as high fever, body ache, joint swelling, and fatigue – much like chikungunya.

Cases of Tomato Flu have currently been reported from Aryankavu, Anchal and Neduvathur in Kerala. Following this, contiguous Karnataka districts – Mangaluru, Udupi, Kodagu, Chamrajnagara and Mysuru – have been directed to keep a vigil on daily travellers from Kerala.

According to the Health Department, tt is also advised to monitor children for any of the aforementioned signs and symptoms in OPDs of health institutions. Further directions have been given to inform the state IDSP section immediately if any cases with such symptoms are detected.

DHOs of concerned districts and other districts too are directed by the Commissioner of Health to ensure surveillance is ensured, the statement said.
